Light work with hand-held tools only (painting, sanding, minor repairs): two (2) business days. A valid Certificate of Insurance must be on file before arrival.

This form must be completed and submitted to management at least five (5) business days prior to the intended start date of any work. Access to the building will not be granted until this form is received and approved.

Weekend work is subject to prior written approval by management and is a privilege that may be revoked if rules are violated.
All water shutdowns must be scheduled at least one (1) week in advance.

Please read and initial each of the following statements:
Elevator Modernization Notice: I understand that 969 Park Avenue is currently undergoing a building-wide elevator modernization project. This will result in one or more elevators being out of service for an extended period. My team and I will plan our work schedule accordingly.
Service Elevator Blackout: I am aware that the service elevator is scheduled to be taken out of service for an extended period during the modernization. During this time, no large material deliveries or debris removal will be possible. All such work must be completed before or after this blackout period.
Elevator Wait Times: I understand that elevator service will be limited throughout the modernization period and that wait times are to be expected. My staff will be instructed to be patient, professional, and courteous at all times.
Staff Escort Requirement: I understand that all workers must be accompanied by a building staff member when using any elevator. Workers may not operate any elevator independently and may not use the stairwells.
Worker Limits: I understand that management reserves the right to limit the number of workers permitted in the building at any one time based on current elevator capacity and building conditions.
No Material Storage: I understand that the building does not provide storage of any kind for contractor materials, equipment, or debris. All materials must be brought in and removed the same day.
Contractor Rules: I have received and read the 969 Park Avenue Contractor Rules and agree to abide by them at all times. Any violation may result in immediate removal from the building and revocation of access.
COI Submitted & Approved: I confirm that a valid Certificate of Insurance has been submitted to and approved by building management prior to the commencement of any work.
Rules Subject to Change: I understand that building rules and access procedures may be updated or amended by management at any time in response to changing conditions during the elevator modernization project. I agree to comply with any updated rules as directed.

Standard Renovation Rules
969 Park Avenue
Standard Renovation Rules
These rules apply to all renovation work performed at 969 Park Avenue. Contractors must sign and return this document before work begins.
1. Work Hours
Standard work hours are Monday through Thursday, 9:00 AM to 4:30 PM, and Friday, 9:00 AM to 4:00 PM. No work is permitted on weekends or public holidays without prior written approval from the Resident Manager. Exceptions for light work (painting, sanding) may be considered on a case-by-case basis.
2. Noise Restrictions
No drilling, hammering, or other high-impact noise is permitted before 10:00 AM. No noise after 4:00 PM. All workers must vacate the building by 5:00 PM Monday–Thursday and 4:00 PM Friday.
3. Elevator Use & Escort Policy
All contractors and workers must be accompanied by a building staff member when using any elevator. Workers may not operate any elevator independently. Workers may not use the stairwells for any purpose. Stairwell B is an emergency exit only. All vertical movement must be coordinated with building staff.
4. Worker Limits
Management reserves the right to limit the number of workers permitted in the building at any one time based on current elevator capacity and building conditions. Limits will be communicated on a week-to-week basis. The current daily cap is 10 workers across all active projects.
5. Service Elevator Blackout Period
During the service elevator modernization period, no large material deliveries or debris removal will be permitted. Only light work and hand-carried materials are allowed during this window. All large deliveries and debris removal must be completed before or scheduled after the blackout period.
6. Hallway Protection
Hallway floors must be protected with Masonite for the full width of the hallway for the duration of any work. Hallway molding must be covered to a height of 4 feet. Hallways must be cleaned at the end of every work day, including wiping down moldings and vacuuming the floor.
7. Apartment Door & Dust Control
The apartment door must remain closed at all times during work. Industrial extractor fans must be running. Walk-off mats must be placed inside the main entry. Plastic zipper or magnetic door liners must be installed.
8. No Material Storage
The building does not provide storage of any kind for contractor materials, equipment, tools, or debris. All materials must be brought in and removed the same day. Nothing may be left in hallways, the lobby, the basement, mechanical rooms, or any common area at any time.
9. Debris Removal
Garbage and debris removal must be scheduled in advance. No debris removal is permitted during the service elevator blackout period. Contractors are responsible for all carting and disposal.
10. Scheduling & Access
All work must be scheduled a minimum of 5 business days in advance. A valid Certificate of Insurance must be on file with management before any work begins. Contractors who arrive without a confirmed appointment or without approved insurance will be denied access.
11. Professional Conduct
All workers must behave in a professional manner at all times. No eating, loitering, or congregating in hallways or common areas. Patience is required when waiting for elevator service. Complaints or confrontations regarding wait times will not be tolerated and may result in revocation of building access.
12. Rules Subject to Change
Building rules and access procedures may be updated or amended by management at any time in response to changing conditions during the elevator modernization project. Management will make reasonable efforts to communicate changes in advance. Contractors agree to comply with any updated rules as directed.
By signing below, I confirm that I have read, understood, and agree to comply with the Standard Renovation Rules of 969 Park Avenue. I understand that violation of these rules may result in immediate removal from the building and revocation of access privileges.

No items to be left in the hallway at any time.
All items must be securely wrapped before leaving the unit.

All workers must act in a professional manner. No loud talking or shouting.
Moves must be scheduled and approved by management in advance after COI review.
